Any user who has complaints or experiences dissatisfaction with the device’s quality, identity, durability, reliability, safety, effectiveness, or performance should notify the distributor or Frontier Therapeutics Limited.
If any Frontier Therapeutics Limited device ever malfunctions and is suspected of having caused or contributed to the death or severe injury, Frontier Therapeutics Limited, your physician, distributor, and your local competent authority should be notified immediately by telephone or written correspondence. When filing a complaint, provide the component(s) name, reference code, LOT number(s), your name, address, the nature of the complaint, and notification of whether a written report from the distributor is requested.
